At Peace Hospice Services, we are dedicated to providing physical comfort, emotional, social, and spiritual support for individuals nearing the end of life. With over 15 years experience and our team of experienced professionals, including doctors, nurses, social workers, and home health aides, all working together to ensure your comfort and dignity during this delicate time.

Hospice is specialized care for life-limiting illnesses. When medicine can only provide comfort, the compassionate staff at LifeChoice will be by your side until you transition. Our mission is to help you pass in peace and have as many meaningful moments as possible in your final days.


Our services include pain management, symptom management, and the medical, physical, social, emotional, and spiritual needs of our clients and their loved ones.


We provide at-home hospice care or can provide our services at a family member’s home, assisted living facility, nursing home, or hospital.


Once your physician confirms your need for hospice care, we work with your doctor and you and your family to create a personalized Plan of Care. You are part of this process, because this is your life, and you choose how you want to spend your final days.


Once your plan is in place, members of our team will visit daily, often more than once per day, to carry out your plan. Your Plan of Care will be adjusted as needed. Our goal is to ensure you are comfortable, minimize pain and suffering, and have the highest quality of life possible.


We have an interdisciplinary team to ensure all of your care needs are covered. Our Medical Director works with your attending physician to determine your in-home medical needs. Your Hospice RN visits you to create a personalized Plan of Care; approved by your physician. Your daily needs are tended to by our Certified Nursing Assistants (CNAs), who report to your Hospice RN—who continues to visit regularly. Volunteers supplement our service providing socialization, our Hospice Chaplain and grief and bereavement counselors are available to you and your family, and your Social Worker can assist with financial planning, advanced directives, and community resources.


Each patient’s needs differ, but we:


  • Create a personalized Plan of Care.
  • Ensure you have the at-home supplies you require—hospital bed, wheelchair, walker, shower stool, transfer aides, etc.
  • Our RN provides your prescriptions and works with your physician to adjust dosage or frequency.
  • Our CNAs assist with activities of daily living—bathing, grooming, toileting, etc.
  • Our volunteers provide social and emotional support and run errands.
  • Our Social worker provides emotional support, support with end-of-life planning, and references to community resources.
  • We can also provide family caregiver respite care, nutritional counseling, access to a non-denominational Chaplain, grief, and bereavement counseling, and more.


Our volunteers’ primary role is to provide social and emotional support and run errands.


Your hospice care is paid for by your insurance provider.


Peace Hospice provides hospice services in your home, a family member’s home, within assisted living facilities, or in nursing homes.


There is no set length of care, but if your care exceeds 6 months your physician will need to recertify your care needs with your insurance company.


Your general care or attending physician must certify that you are terminally ill and likely have 6 months or less to live.


We provide care in the following counties;

Florida

Baker, Clay, Duval, Flagler, Nassau, Putnam, St. Johns.
